This is the official Australasian Menopause Society Symptom Score Card using the validated Greene Climacteric Scale. This tool can document symptoms and monitor treatment response but should NOT be used to diagnose perimenopause or menopause.

Results should be interpreted by qualified healthcare professionals as part of a comprehensive clinical assessment.

Menopause Symptom Assessment

Please rate how much each symptom has bothered you during the past month. Rate each symptom from 0 (not at all) to 3 (extremely).

Psychological Symptoms

1. Heart beating quickly or strongly

Episodes of rapid or forceful heartbeat

2. Feeling tense or nervous

General feelings of tension or nervousness

3. Difficulty in sleeping

Problems falling asleep or staying asleep

4. Excitable

Feeling easily excited or overstimulated

5. Attacks of panic

Episodes of sudden intense fear or anxiety

6. Difficulty in concentrating

Problems focusing on tasks or thoughts

7. Feeling tired or lacking in energy

Persistent fatigue or low energy levels

8. Loss of interest in most things

Reduced interest in activities you used to enjoy

9. Feeling unhappy or depressed

Persistent feelings of sadness or depression

10. Crying spells

Episodes of crying or feeling tearful

11. Irritability

Feeling easily annoyed or frustrated

Physical Symptoms

12. Feeling dizzy or faint

Episodes of dizziness or feeling like you might faint

13. Pressure or tightness in the head or body

Sensations of pressure or tightness

14. Parts of body feeling numb or tingling

Numbness, tingling, or pins and needles sensations

15. Headaches

Frequency and severity of headaches

16. Muscle and joint pain

Aches and pains in muscles and joints

17. Loss of feeling in the hands or feet

Reduced sensation in hands or feet

18. Breathing difficulties

Problems with breathing or shortness of breath

Vasomotor Symptoms

19. Hot flushes

Episodes of sudden heat and sweating

20. Sweating at night

Night sweats that may disturb sleep

Sexual Function

21. Loss of interest in sex

Reduced interest in sexual activity
