You have probably calculated your BMI at some point. Enter your height, enter your weight, get a number. But what does that number actually mean for your health — and what does it miss?

Body Mass Index remains one of the most widely used tools for categorising weight in adults. It is simple, inexpensive, and offers useful population-level insight. But used in isolation, it tells an incomplete story.

At Downscale Health, we believe the more useful approach is understanding what BMI can tell you — and what it genuinely cannot.


What Is BMI?

BMI stands for Body Mass Index. It is calculated by dividing your weight in kilograms by your height in metres squared:

BMI = weight (kg) ÷ height² (m²)

For example, a person weighing 80 kg who is 1.70 m tall has a BMI of approximately 27.7.

The following categories are widely referenced by the Australian Government Department of Health, Disability and Ageing, the Australian Institute of Health and Welfare (AIHW), and Healthdirect Australia:

Where BMI Is Genuinely Useful

BMI is a reasonable starting point — particularly at a population level. It provides a standardised way of relating weight to height, making it practical for tracking weight trends across large groups over time.

The AIHW describes BMI as a tool designed primarily for population health monitoring, while also acknowledging its limitations when applied to individuals. In clinical practice, it can serve as one useful data point when considered alongside other information.

The problem arises when we expect a single number to carry more diagnostic weight than it can.


What BMI Does Not Tell You

Two people can share an identical BMI and have substantially different bodies and health profiles. Here is what BMI cannot reveal:

How much of your weight is muscle. BMI is derived from total body weight and does not directly measure body fat or distinguish muscle mass from adipose tissue. A highly muscular person may be classified as overweight or obese without carrying excess body fat.

Where your body fat is stored. Fat distribution matters enormously. Visceral fat — fat stored around the abdominal organs — is associated with significantly increased cardiometabolic risk, including type 2 diabetes and cardiovascular disease. BMI cannot show where fat is distributed in your body. Waist circumference provides important additional information here.

Your metabolic health. BMI says nothing about your blood pressure, fasting glucose, HbA1c, cholesterol, triglycerides, or other markers that directly inform cardiovascular and metabolic risk assessment.

Your fitness and physical capacity. A BMI result does not reflect aerobic fitness, muscular strength, or physical activity levels — all of which have independent effects on health outcomes.

Your nutrition, sleep, mental health, or history. A number derived from two measurements cannot capture the quality of your diet, the adequacy of your sleep, your psychological wellbeing, your medical history, your medications, or your genetics.

And it says absolutely nothing about your worth as a person.

That is a great deal to expect from a simple mathematical equation.


Why Waist Circumference Matters

Waist circumference is one of the most clinically valuable measurements to consider alongside BMI. It provides an indication of abdominal fat distribution that BMI simply cannot offer.

Australian guidance recommends measuring waist circumference alongside BMI because elevated waist circumference is independently associated with increased risk of type 2 diabetes, cardiovascular disease, and metabolic syndrome.

As a general guide, health risk increases at:

  • Men: above 94 cm, with substantially higher risk above 102 cm

  • Women: above 80 cm, with substantially higher risk above 88 cm

Individual clinical context always applies. This is a practical example of why one measurement rarely tells the complete story.


Does BMI Work Equally Well for Everyone?

No. Standard adult BMI classifications have recognised limitations across several groups.

  • BMI can misclassify people with high lean mass, including some athletes.

  • Age and pregnancy status can affect how results should be interpreted.

  • Ethnic background matters — some research suggests that people of Asian background may carry metabolic risk at lower BMI thresholds than standard classifications reflect.

  • BMI is not interpreted the same way for children and adolescents, where age- and sex-specific growth charts are used instead of adult cut-offs.

These are further reasons why a BMI result should be interpreted in clinical context, not treated as a standalone diagnosis.


What Should Be Considered Alongside BMI?

A thorough clinical assessment of weight and health may include a range of factors depending on the individual:

  • Waist circumference and body fat distribution

  • Blood pressure

  • Fasting blood glucose, HbA1c and insulin resistance markers

  • Cholesterol and triglycerides

  • Liver function and relevant pathology

  • Medical and family history

  • Current medications

  • Sleep quality and patterns

  • Nutritional intake and eating behaviours

  • Physical activity levels and fitness

  • Muscle mass and strength

  • Hormonal and reproductive health

  • Mental health and psychological wellbeing

  • Weight history and previous management attempts

Not every individual requires every assessment. The point is that health is considerably larger than any single number.


Should You Know Your BMI?

Yes — as one piece of information among many. Think of BMI as a starting point, not a verdict.

Knowing your BMI helps you understand where your weight sits relative to your height using standard adult classifications. What that means for your individual health depends on a broader picture: your body composition, waist circumference, pathology results, lifestyle, medical history, and personal goals.

Can BMI be high because of muscle?

Yes. Because BMI does not distinguish between muscle and fat, a person with substantial lean mass can have a higher BMI without carrying the same degree of adiposity as someone else with an identical result.
