What medical weight management actually is

Medical weight management is weight care delivered by a clinician, built on an assessment of why your weight is where it is rather than on a diet you are handed. It looks at hormones, metabolism, sleep, mood, medicines and the pattern of what has happened before, then builds a plan around what it finds and reviews it as things change.

The difference from a programme is that nothing is decided before you are assessed. Two people the same height and weight can arrive with entirely different reasons for being there — one with an untreated thyroid problem and broken sleep, another whose weight tracked a medicine they were prescribed for something else. The same plan would fail at least one of them, which is why a plan comes second.

Why this is treated as a medical condition

Obesity is recognised clinically as a chronic, relapsing condition with biological, hormonal, psychological and environmental drivers. The body defends a weight it has settled at, and defends it harder the more aggressively it is pushed. That is the mechanism behind regain after a successful loss, and it is physiology rather than a failure of resolve.

Treating it as a medical condition changes what the appointment is for. The question stops being how much more effort to apply and becomes which mechanism is involved and what can be done about that one.

What a first appointment covers

A full history: your general health, the medicines you take, your sleep, your mood, your eating and activity patterns, and what you have already tried and what happened. Where it is useful, your clinician will arrange pathology so the conversation is built on results rather than assumptions. We have set out the blood tests worth asking for if you would rather take that list to your own GP.

Where treatment is clinically appropriate, your clinician discusses the potential benefits and the potential risks with you, and the decision is made together. Nothing is automatic and nothing is one-size-fits-all.

What it is not

It is not a meal plan, a challenge, or a subscription that sends you the same thing it sends everyone. It is not a promise about a number, because no honest clinician can make one. And it is not a single appointment: weight care is ongoing by nature, and the value is in the reviews as much as the start.

If your weight has stopped moving

That is usually a signal rather than a failure, and it is worth understanding before you try harder at the same thing. Why can't I lose weight? sets out the nine most common medical reasons and the tests that tell you which one applies to you. It is free to read and there is no account.
