Clinical services
Care at Downscale is one plan drawing on six connected areas, not six separate products you choose between. Which of them your plan leans on depends on what your assessment shows, and it changes over time as your situation does.
Everything below is delivered by telehealth, anywhere in Australia, one-to-one with a registered nurse practitioner. For what we treat rather than how, see conditions we treat.
Medical weight management
Clinical assessment of why your weight is where it is — hormones, metabolism, sleep, mood, medicines and history — and a plan built around what that shows, reviewed as things change.
Nutrition and meal planning
Eating patterns you can hold, built around your life rather than a template. Not a meal plan you are handed and expected to follow forever.
Movement and activity
Activity that fits the body and the week you actually have. Resistance work matters here more than most people expect, because muscle is what keeps resting energy use up.
Mental health support
Mood, stress, habits and weight are connected in both directions. Support for the part of this that is not about food.
Sleep and recovery
One of the most overlooked drivers of metabolic health. Short or broken sleep raises appetite hormones and lowers the ones that signal fullness, night after night.
Goal setting and maintenance
Keeping weight off is a different task from losing it, and it is the one most programmes stop supporting. Maintenance is where the ongoing care earns its place.
How a plan is put together
Assessment first. A full history, the medicines you take, your sleep, your mood, your patterns, and pathology where it is useful. The plan follows from that rather than preceding it, which is why two people the same height and weight will not get the same one.
Reviews are where most of the work happens. Weight is a moving target and so is the rest of your life, so the plan is revisited rather than handed over once. That is also the part that is hardest to sustain in person, and the reason telehealth suits this particular kind of care.
If your weight has stopped moving
Before booking anything, it is worth understanding why. Why can't I lose weight? covers the nine most common medical reasons, and the blood tests worth asking for is a list you can take to your own GP. Both are free and neither requires an account.
